Speech Improvement Class Forms
This document contains the streamlined forms used by San Diego Unified School District for the Speech Improvement Class. All documents are copyrighted to San Diego Unified School District. If you would like permission to use these forms, please contact me. In this document, you will find:
1. Describing Speech Misarticulations - This teacher questionnaire enables SLPs to determine if general (SIC) or special education is warranted.
2. Permission to Enroll - This form is signed by the family (in English & Spanish) prior to enrollment.
3. SIC Checklist - This includes all tasks that need to be completed throughout Speech Improvement Class.
4. Homework Contract - This contract underscores the importance of home practice (in English & Spanish).
5. Participating/Completion Form - This form is placed in the child's cumulative file to document general education services (one for during class and one for after completion).
6. Sample Speech Improvement Sound Inventory for /s/ (all sound inventories available for download below) - This tool is employed for progress monitoring in addition to conversation samples.
7. Sonority Sequencing Principle for Clusters - This is based on a research-based principle for selecting complex cluster to teach when three-element clusters are not a viable target.
8. Summary of Best Practices - This document summarizes the research that provides the basis for San Diego Unified's Speech Improvement Class.
