This teacher questionnaire enables SLPs to determine if general (Speech Improvement Class) or special education is warranted according to California Education Code. The questions allow the SLP to determine whether or not the child meets eligibility criteria. In some cases, it may be advisable to use this questionnaire to interview a teacher to elicit the most accurate description of a child’s sound production.
Please note many states and districts may have different criteria for special education eligibility. These flow maps feature San Diego Unified’s interpretation of California Education Code (5CCR3030), which stipulates that a child must meet all three of the following criteria: 1) the disorder has a significant impact on intelligibility, 2) the disorder attracts adverse attention, and 3) the disorder significantly impacts educational performance.
