This document serves as a report template for SLPs to fully characterize a child’s phonological and overall communication in family-friendly terms. It includes both independent analyses and relational analyses. In addition, the report stipulates the three criteria for sound production IEPs according to California Education Code. SLPs can provide evidence that a child does (or does not) meet each criterion.
