Full Assessment Request

In San Diego Unified, full assessments are completed by the Special Education Early Childhood (SEEC) Assessment Teams. The referring SLP and teachers from the preschool site are strongly encouraged to provide information about the child’s strengths, areas of concern, previous interventions and progress monitoring. SLPs are required to complete this form and to provide relevant information during the process. Performance-based assessment consists of authentic data through observations across contexts, behavior sampling (e.g., language samples) and parent and teacher interviews. The emphasis is on the child’s strengths and potential for growth over time.
