Permission to Enroll Form
This permission form is signed by the family prior to enrollment in the Speech Improvement Class. It describes the class as a general education service that generally lasts 17-20 hours and stipulates that home practice is required for participation. It is currently available in English, Spanish, Somali, and Vietnamese.
